BSEE to Investigate Death on Fieldwood Energy Offshore Platform

BSEE has announced an investigation into the May 15, 2021 fatality at a Fieldwood Energy offshore platform in the Gulf of Mexico.  During the incident, a contractor on Fieldwood’s Eugene Island 158 #14 platform. lost his life during a casing pressure test on a shut in well. The Bureau of Safety and Environmental Enforcement (BSEE) has formally announced it has formed a panel to investigate this fatality.  A team of inspectors will investigate the incident, and ultimately issue a report containing findings as to the cause.  Recommendations will likely also be issued.
