The Bureau of Safety and Environmental Enforcement (BSEE) has released its investigation report on the November 19, 2022 allision when the vessel Jacob Gerald struck the West Delta (WD) 31 Platform “E-DRL” Platform in the Gulf of Mexico. At the time of the incident, the vessel was transporting personnel to the platform when it struck it. One production operator was transported to shore for medical care.
The BSEE report determined the probable cause of the incident:
- Human Performance Error – Inattention to task/Fatigue: Captain fell asleep at the helm.
