A tragic accident early Tuesday, April 29, 2025 at the Port Arthur LNG construction site in Sabine Pass, Texas claimed the lives of three workers and left two others injured. The scaffold collapse occurred around 2 a.m. at the large industrial facility, where hundreds of workers are on-site around the clock.
According to reports, the incident happened during work on a major section of the project, overseen by engineering and construction contractor Bechtel. At this time it is believed a tank jump form system failure at an elevated position caused the deadly collapse. Two workers were declared dead at the scene. A third victim, identified as a 41-year-old man from Houston, died later at Christus St. Elizabeth Hospital in Beaumont after suffering cardiac arrest. The two other workers injured in the collapse were initially hospitalized but have since been released.
Investigation Underway Into Scaffolding Failure
Federal investigators from the Occupational Safety and Health Administration (OSHA) have launched an investigation to determine what led to the collapse. The precise cause remains unclear, and questions persist about whether the workers were using proper fall protection gear at the time.
From vantage points near the site, witnesses observed a significant section of the scaffold — spanning what appeared to be three levels — separated and tilted sharply away from the structure. Observers also reported seeing a dangling rigging strap from a crane, suggesting that a load might have dropped and impacted the scaffolding, although officials have not confirmed if rigging failure contributed to the accident.
Safety Concerns on Major Industrial Projects
Large-scale industrial projects like the Port Arthur LNG site involve complex, high-risk construction activities. Scaffolding failures are particularly dangerous, often resulting in severe injuries or fatalities due to falls from height. Potential causes can include improper assembly, overloaded structures, poor housekeeping, rigging mistakes, or the use of faulty materials.
Ensuring strict adherence to safety protocols, routine inspections, and proper use of fall protection equipment are critical measures needed to reduce these risks. As the investigation continues, regulators will be examining whether safety standards were upheld and if additional preventative measures should have been in place.
