NTSB Completes Investigation of Deadly Madisonville, Texas Plane Crash

The NTSB has completed its investigation following the deadly crash of a Piper PA-32 in Madisonville, Texas on June 14, 2021. During the accident, the plane crashed near the local regional airport while attempting to land.   According to the Texas Department of Public Safety, the pilot, Apolo Diaz, is the lone fatality.  Five passengers were injured in the crash.  The plane appears to have struck trees while attempting to land, and crashed 400 feet short of the runway.  The Madisonville Airport is an unmanned airport, which is supposed to have lights to guide approaching aircraft during nighttime hours.  It is unknown at this time if the guide-lights were in use at the time of the crash.

There have been a number of recent plane crashes in 2021.  A Cirrus SR-20 crashed near Truckee, California on June 15, 2021. One person was killed and one critically injured in a plane crash in Gila Bend, Arizona on June 15, 2021.The pilot died in a small plane crash in Bucks County, Pennsylvania on June 15, 2021. A Piper PA32-260 crashed in Madisonville, Texas on June 14, 2021.  Two people were injured when a Piper 140 crashed near the Denton, Texas airport on June 9, 2021.  There was a small plane crash in Collier County, Florida on June 7, 2021.  A pilot was killed when a Vans RV6A crashed in Porterville, California on June 7, 2021. There was a small plane crash in Denton County, Texas on June 4, 2021.  A Beechcraft A23 crashed in a field in Pinnacle, North Carolina on June 1, 2021.  There was a small plane crash in Pearland, Texas on May 29, 2021.
