Fishing Boat Explosion caused by Welding Equipment

The Honolulu Fire Department has finished its investigation into the explosion on the fishing boat KIM THU on April 26, 2024.  An explosion occurred on the fishing boat while it was docked at Pier 36 in Honolulu, Hawaii.  Five crew members were burned during the explosion.  Following an investigation, the HFD believes that welding equipment near the vessel ignited diesel vapors from the vessel’s fuel tank.

Welding and other hot work activities on vessels present a significant risk of explosions, particularly due to the ignition of diesel vapors, which are commonly present in marine environments. Diesel is often stored and used on vessels for fueling engines and other equipment, creating a constant presence of flammable vapors. When hot work is conducted, the high temperatures involved can easily ignite these vapors. The risk is particularly acute in enclosed or poorly ventilated spaces where diesel vapors can accumulate, such as engine rooms, fuel storage compartments, and under decks. Such environments can quickly become highly explosive if any source of ignition, like sparks from welding, comes into contact with concentrated diesel vapors.

The nature of welding adds to the complexity and danger. Welding equipment generates sparks and produces a substantial amount of heat. Even with the completion of welding, the residual heat can remain high enough to ignite flammable vapors or materials in the vicinity long after the active work has stopped. The risk is compounded by the potential for sparks to travel significant distances from the original work area. These sparks can find their way into unseen crevices or through small openings where vapors may be concentrated, setting the stage for a sudden and violent explosion if conditions are right.

Safety measures are critical to managing these risks and must be rigorously implemented on vessels. This includes conducting thorough risk assessments before any hot work begins to identify and mitigate potential hazards. Areas where hot work is to be performed must be tested for flammable gases, and continuous monitoring should be in place to detect any changes in the atmosphere. Adequate ventilation is essential to disperse harmful vapors and reduce their concentration in the air, thereby lowering the risk of ignition. Additionally, all equipment and tools should be checked to ensure they are in good working condition and do not pose any additional risks.

Training and preparedness are also vital components of safety protocols in these environments. Crew members involved in or around hot work operations should be trained not only in proper welding techniques but also in emergency response procedures. Clear communication about the start and end times of hot work activities should be maintained to ensure all personnel are aware and can take appropriate precautions. Furthermore, effective firefighting equipment must be readily available and accessible, with crew members trained in its use should an outbreak of fire occur. By adhering to these safety practices, the risk of explosions due to the ignition of diesel vapors during hot work can be significantly reduced, protecting both the vessel and its crew.
